STEP 1

Complete the enrollment form and the medical questionnaire. You will be asked for a Participant ID. Please use your employee ID # as your Participant ID.

FIRST TIME REGISTRATIONS:  If you are completing the enrollment form for the FIRST time, click on the 'Continue' button after clicking on "Enroll Now" below.

RETURNING EMPLOYEES ONLY: If you are a RETURNING participant, please be sure to use the SEARCH feature on the next page to locate yourself in the system.

STEP 2:

Physician Name is not required however the field can not be left blank.  If you do not have a Physician, please fill in the blank with NA.

STEP 3:

Under "Group" please choose your work site.
