We are very pleased that you have decided to take advantage of this wonderful opportunity to become better informed about your health. This health questionnaire is designed to accompany the “Be Well at Shell” Premier Prevention Exam Program offered by SHELL.  Please note that it takes approximately five minutes to complete this questionnaire. It must be completed in one sitting.

Please complete the online questionnaire prior to you arriving for your “Be Well at Shell” Premier Prevention Exam at Kelsey-Seybold Clinic in order to receive:

  •  an assessment of your personal risk for the development of coronary heart disease, type 2 diabetes, stroke, congestive heart failure, cancer (breast or prostate and colorectal)

To register for your Know Your Number™ health assessment, please follow these simple steps.

STEP 1:

 After reading these instructions, click Enroll Now below.  Again, once you begin the process, you must finish it at one sitting.  You cannot return to complete your questionnaire at a later time.

STEP 2:

Complete the enrollment form and the medical questionnaire.  You will be asked for a Participant ID.  If this is your first time to register, we recommend using your Employee ID number or something that is easy for you to remember. 

STEP 3:

At your Premier Prevention Exam, the required blood panel testing and the clinical measurements will be done.  The completed Know Your Number™ health assessment will be mailed to you, along with your “Be Well at Shell” exam results and physician letter no later than 7-10 business days after your scheduled appointment.

Please do not consume any food or drink (water and decaffeinated black coffee – with no sugar, cream or additives – are acceptable) for 12 hours prior to your appointment.  Please take any regularly prescribed medications with water ast you normally do.
